General Release and Waiver Claims

By registering and participating in this program hosted by Alpha Kappa Alpha Sorority, Incorporated® Alpha Alpha Chi Omega Chapter, you agree to the following General Release, Waiver of Claims, and other provisions:

I acknowledge that participation in the Get Pretty Fit w/ AAXO program exposes me to possible personal injury; property damage; and/or related losses. Therefore, I release Alpha Kappa Alpha Sorority, Incorporated® Alpha Alpha Chi Omega Chapter, its partner organizations, contractors, employees, and members from all liability from any and all liability from injury, damage, loss, or other claims arising from or in connection with my participation in the Get Pretty Fit w/ AAXO program, including claims that are known and unknown, foreseen and unforeseen, future or contingent, arising before, during or after this Get Pretty Fit w/ AAXO program. This release includes potential damage, losses or claims arising from food or beverage consumption.

  I authorize Alpha Kappa Alpha Sorority, Incorporated® Alpha Alpha Chi Omega Chapter, its partner organizations, contractors, employees, and members to photograph, interview, video and/or audio record me and use the resulting product in any medium (including but not limited to print, electronic (web-based), and/or social media) for educational, promotional and marketing purposes. This authorization shall extend to all current and later discovered languages, media, formats and markets. This authorization shall continue indefinitely unless revoked by me in writing and delivered. I waive my right to inspect or approve any product in which my likeness is used. I will receive no compensation and will make no claim in connection with the use of my name, likeness, interviews, and/or quotes.

I acknowledge that Alpha Kappa Alpha Sorority, Incorporated® Alpha Alpha Chi Omega Chapter reserves the right to remove any displays or deny my entry to any event.

I agree to adhere to all instructions, guidelines, and safety rules.

I acknowledge that no refunds or rain checks shall be issued for any reason including due to inclement weather.

I fully understand and agree to the foregoing provisions. I understand that this document serves as an agreement enforceable against me, my heirs, legal representatives, guardian, executors, successors, and assigns in a court of law.
